Eligibility
Wisconsin-based organizations working towards health equity or addressing social determinants of health. Priority for organizations with less than $500,000 in annual expenses or that obtained nonprofit status after January 1, 2021.
Requirements
- Must be a Wisconsin-based organization
- Must work on health equity or social determinants of health
- Priority for organizations with under $500,000 in annual expenses
