Eligibility
Oregon-licensed physicians (MD, DO, ND) in family medicine, general practice, internal medicine, geriatrics, pediatrics, or OB/GYN, as well as dentists and pharmacists practicing in health professional shortage areas or medically underserved areas.
Requirements
- Must hold valid Oregon clinical license in eligible discipline
- Must serve patients in a health professional shortage area or medically underserved area
- Minimum 3-year service commitment required
