About this program
Jersey Assistance for Community Caregiving provides in-home and community-based services to eligible New Jersey residents age 60 and older who require nursing home level of care but want to remain at home. Deadline notes: Applications are generally handled through county Aging and Disability Resource Connection offices. Notes: Core New Jersey aging-in-place support program.
Eligibility
New Jersey residents age 60 or older who meet clinical eligibility for nursing home level of care, financial criteria, and program requirements, and who are not enrolled in certain overlapping Medicaid or state programs.
Requirements
- Contact the county ADRC for assessment and provide age, residency, income, resource, medical, functional need, and care plan information.
